• D2S Waiver -  REQUIRED This is the waiver required by D2S ministries to go on the trip.



• D2S Blaze Schedule - INFORMATION This is the informational packet provided by D2S ministries.



• D2S Blaze Info (Church Packet) - INFORMATION entails where we will be staying, when we are leaving, and when we will be arriving back.



• D2S Medical Release (Church Packet) - REQUIRED This is the Medical Release/Waiver form provided by the Church that is required to go to D2S with Face to Face Youth.
